Parent and student information is used only for school operations.

This template is configured for education programs that collect information from parents or guardians, never through student self-entry.

What is collected

Inquiry, booking, and registration forms collect parent/guardian contact details, child age or grade, program interest, scheduling notes, consent status, SMS opt-in, and photo permission when applicable.

How it is used

Staff use this data to respond to inquiries, schedule consultations or trial classes, review applications, manage attendance, prepare lesson summaries, and track tuition status. The system does not process online payments.

Consent controls

Parent data consent and terms acceptance are required before submission. SMS reminders are opt-in only. Photo permission is separate and remains unchecked unless a parent or guardian grants it.

Access and deletion

Verified guardians may request export or deletion review. Staff must confirm identity and operational retention needs before completing deletion.
